For the use of Rosemont Health & Fitness (hereinafter referred to as "RHF"), the Guest (hereinafter referred to as "Releasee") expressly agrees that RHF shall not be liable for any damages arising from personal injury for use of any equipment or its improper maintenance, use of any exercise equipment which may malfunction or break, RHFs improper maintenance of any exercise equipment, RHFs negligent instruction or supervision, RHFs negligent hiring or negligent retention of any employee, loss of consortium, slipping and falling while in RHF, or on the surrounding premises, or first aid, emergency treatment or any other services which are negligently rendered or failed to be rendered by RHF, emergency personnel or Good Samaritans, or negligently preventing a Good Samaritan from rendering first aid to the Releasee, in or about the premises of RHF as a result of the use of RHF and the equipment therein. By the execution of this agreement, Releasee assumes full responsibility for any such injuries or damages which may occur to the Releasee in or about RHF, and further agrees that RHF shall not be liable for any loss or theft of personal property. Releasee assumes full responsibility for any such injuries, damages or losses which may occur to the Releasee in or about RHF facility, and does hereby fully and forever release and discharge RHF and the Village of Rosemont, their owners, employees and agents from any and all claims, demands, damages, rights or action, present or future, whether the same be known or unknown, anticipated or unanticipated, resulting from or arising out of Releasee's use or intended use of RHF facilities and equipment thereof.

I further stipulate and agree, while utilizing said facilities and equipment, to be bound by all orders, rules and regulations of RHF.

In consideration o receiving permission from Rosemont Health & Fitness (hereinafter referred to as "RHF") lo allow its Members/Guests children who are over the age of sixteen (16)* access to and use of RHF, the Member/Guest expressly agrees that RHF shall not be liable for any damages arising from personal injury because of the use of RHF, or its improper maintenance, use of any exercise equipment which may malfunction or break, RHF's negligent hiring or negligent retention of any employee, slipping and falling while in RHF or on the surrounding premises, or first aid emergency treatment or any other services which are negligently rendered or failed to be rendered by RHF emergency personnel or Good Samaritans, or are negligently preventing a Good Samaritan from rendering first aid sustained by the Members/Guests minor child, in or about RHF, or as a result of their using RIF facilities and the equipment herein. By the execution of this agreement, Member/Guest assumes full responsibility for any such injuries or damages which may occur to the Members/Guests minor child in or about RHF and does hereby fully and forever release and discharge RHF, the Village of Rosemont, their owners, employees and agents from any and all claims, demands, damages. right or action or causes of action. present or future. whether the same be known or unknown, anticipated or unanticipated, resulting from or arising out of Members/Guests minor child's use, or intended use of RHF. or the facilities and equipment thereof. *All age access on family fitness Days.

Member/Guest stipulates and agrees, on behalf of said minor child. that while utilizing said facilities and equipment, said minor child will be bound by all orders, rules and regulations of RHF.
